Home Program / Hospital Program
Admission to the Palliative Care Program is by medical referral.

The Home Program provides coordinated care within the security of the client's home.  Based upon the client's needs, community nursing; home support; physical and occupational therapy; emotional and nutritional counseling are available.

The Palliative Care Team is an interdisciplinary collaborative group of health care providers who work with the client's physician.  Their goal is to provide clients with services addressed to the client's specific needs.  The Palliative Care Team meets weekly to address care plans for clients in a variety of settings and monitor their progress.

Some medical supplies and equipment needed for care at home are supplied at no charge, i.e. hospital bed, chair lift, wheelchair, walker, pressure relief mattress, bath chair, commode, routine supplies for dressings, IV therapy, urinary catheters, and incontinence.

Clients may require admission to acute care if their care needs cannot be met at home.
