Administration
• Board is Advisor Committee to Interior Health Authority and Program Coordinator
• Staff report to Home and Community Manager
• Program Coordinator - .76 FTE
• Bereavement and Education Coordinator - .20 FTE
Volunteer Program
Volunteers provide support to community, LTC facility and hospital; primarily in palliative phase but can also do
bereavement support.
Home Program
Liaise formally with Home Nursing Care at monthly Core Team meeting which includes Social Worker, Oncology Nurse, Home Care Nurse, Patient Care Coordinator and Bereavement Coordinator.
Hospital Program
Liaise & Attend both formal & informal meetings with Interdisciplinary meetings
Free Standing Hospice
Feasibility study underway to assess whether this is the direction we should go.
Spiritual and Pastoral Care
Liaise formally with Spiritual Care Team as well as strong network with religious community.
Bereavement Program
Bereavement Coordinator provides supportive counselling, and community support groups.
Education and Professional Training
• Education, support and advocacy available to community as requested (ie. critical incidents)
• Events: “How to Get Thru Christmas”, “Living Thru Grief” Conference
Evaluation and Research
Feasibility study to evaluate palliative care services in community underway.
Organization Mission Statement
We support quality of living while in the process of dying, and healthy transition through grief for the bereaved.
