BCHPCA Components of Care

Administration
Partnership between Chilliwack General Hospital, Home Health Care, and the Chilliwack Hospice Society.  Funded by the Fraser Health Authority.  Donations accepted.  Fund raising done by Chilliwack Hospice Society.

Home Program
• Integrated interdisciplinary program
• Weekly community based rounds
• 24-hr phone access to collaboration with hospitals
• Supplies and equipment through BC Palliative Benefits Program Palliative Care Communication
• Binder for families

Hospital Program
• Designated beds on a medical unit
• One respite bed in extended care
• Weekly interdisciplinary rounds
• 24-hr phone access

Spiritual Care
Spiritual Care Coordinator at Chilliwack General Hospital is a member of the consultative team (0.5FTE)
Partnership with Chilliwack Ministerial Association (providing visiting/on call services)

Symptom Management
Interdisciplinary team provides consultation re symptom management in a variety of settings (i.e. home, hospital, residential care).  Sessional palliative care physician support Psychosocial support bereavement follow up.

Bereavement Program
• Mail outreach and telephone follow up by Hospice Volunteers
• Hospice 1:1 Support Grief support groups
• Counselling/referrals as indicated

Education and Professional Training
• Orientation and Continuing Education programs
• Symptoms Management Guidelines
• Palliative Care Communication Binder for families
• Collaboration with the University College of the Fraser Valley (Nursing Program) and the UBC Family Practice Residency Program (Chilliwack site).
