This award program is generously funded by the Sovereign Order of St. John of Jerusalem, Knights Hospitaller and administered by BCHPCA. Awards will be granted at the BCHPCA Annual Conference May 13-15, 2005. The awards, given in the form of grants, are intended to supplement direct care to patients and families at the end of life. The grants are not intended to cover administrative or infrastructure costs.
Program members of BCHPCA who are in good standing can apply for the awards. The Awards Committee anticipates making 4-5 grants available, with each grant valued at $1000 to $2000.
Applications for funding must be received no later than April 1, 2005.
To apply for a grant, please submit a 2-3 page letter of intent outlining the
-objectives of the planned initiative
-identification of the target audience
-description of how the target audience will benefit
-outline of how you evauate success.
